How to file a petition for correction in the child's LAST NAME in his/her Certificate of Live Birth?

What if the last name appearing in your Birth Certificate is wrong?

Take this example:

-Juana is a daughter of Reynaldo Guevarra and Editha Enriquez. 

-Hence, Juana's full name should appear as JUANA ENRIQUEZ GUEVARRA in her Birth Certificate. 

-But in her Birth Certificate, her name appears as JUANA ENRIQUEZ GUEVARA.

-Note that her last name is incorrectly spelled, that is, GUEVARA  instead of GUEVARRA. 

This error, as well as any other error in the spelling in  the last name of a child, can be corrected through a filing a PETITION FOR CORRECTION OF CLERICAL ERROR IN THE CERTIFICATE OF LIVE BIRTH in your nearest local Civil Registry Office.

What are the requirements?
a. Birth Certificate for correction
b.1 Birth Certificate / Death Certificate of father (if child's last name is from                                                     his/her father, i.e.,legitimate or acknowledged children)
b.2 Birth Certificate / Death Certificate of mother (if child's last name is from                                                   his/her mother, i.e.,illegitimate or natural children)
c. Marriage Contract of parents ( if parents are married)
d. Birth Certificate of siblings (brothers/sisters)
e. Other documents / IDs that might be required by the local registrar

It should be noted that different Civil Registry Offices would sometimes vary in their requirements.

Zamboanga City Civil Registrar's Office would only require atleast three (3) of the said documents.

RA 10172 (Correction of Sex and Date and Month of birth in the Certificate of Live Birth)

An Act Further Authorizing the City or Municipal Civil Registrar or the Consul General to Correct Clerical or Typographical Errors in the Day and Month in the Date of Birth or Sex of a Person Appearing in the Civil Register Without Need of a Judicial Order, Amending for this Purpose Act Numbered Ninety Forty-Eight.


Good news! Error in the SEX and DATE and MONTH of birth in the Certificate of Live Birth, can now be corrected in the Civil Registrar's Office. This means, goodbye to lengthy and expensive court petitions, court hearings and court proceedings!


As a processing clerk for RA 10172 and RA 9048, i have to deal with these matters 5 times a week, 8 hours a day!

Comfort in every sniff

Been drinking cold medicines, but seems like i'm not getting the relief i need. 

Good thing I have Vicks VapoInhaler for Nasal Congestion Relief on hand, it provides just the immediate aid I am yearning for. Though it just gives a temporary relief for my nasal congestion, i'm still so thankful that every sniff lightens my stuffy nose.

I bought mine, like 3 months ago and its still usable. Its really handy and can be used almost everywhere.  Why not get one now?

colds... you're keeping me down!

Imagine this:
On my office table are piled up documents begging for my attention. Approved documents need to be forwarded to other division. Clients scheduled for today came just to find out that they have to come again tomorrow since the processing clerk cant make it to work. 

These are some of the scenarios at the office today! I tried but i really cant report today. My colds are strangling me on my bed.

As the rainy seasons came, i should have been more cautious. Since I work in a public service office, and lots of people are having colds and cough, I know I should have taken precaution more seriously! 

I was just wondering, why do i have colds, when in fact i just got a shot of Influenza Vaccine on the 29th of last month!! Is colds not included in that vaccine???? Here are some pics of that:




Anyways, fact is, i'm have colds and i have to deal with it! Now, im GOOGLING on how to get rid of it fast. And here's what i got from wiki how:

In the beginning of your cold you will experience minor symptoms such as running nose, scratchy throat, and minor tiredness. To treat these symptoms and prevent them from reaching the next stage, follow these tips.
  • For the running nose, first get a pot, towel, stove and water and then boil the water in the pot. Once boiling put a lid on it and let it boil for 5 minutes. After 5 minutes remove the lid. Place your head over the steam and then drape a towel over your head and the pot so that the steam stays where you want it to. Breath in deeply and breath out gently. Do this for about 15 minutes every day about 7 times a day and also include at least 2 showers. 1 for mornings and 1 for nights.
  • If possible do this using an inhaler. It is a simple electric kettle for boiling water, with a hole in the lid, so that steam can be inhaled while the water boils. It is safer than bending over a stove, head covered with a towel. Keeping the towel away from the flame (if using a gas stove) can be a challenge if you are focused on inhalation.
  • For the scratchy throat purchase some club soda (also known as sparkling water) and drink as much as you can. Also gargle with salt water 2 to 3 times a day (recommended that it's gargled with after each meal of the day).
  • For the minor tiredness try to get at least 12 hours of sleep for the night and drink black tea. Black tea is the tea that usually has the most caffeine it is still light though and its still healthy for you.
  • If black tea is unavailable drink Green tea. Also try drinking any warm fluid in general as well as drinking a lot orange juice and of course, plenty of water.

Ok, so now i have to gather a pot, water, towel, club soda and get lots of sleep... i hope this will do the trick! Get well soon to me!

Just got a jump start to my so-called long-planned-life-corrector routine.
What I am actually trying to say is, today i took my first big step to whitewash my frustrations. Im not really that open yet for these life-insecurities to be known to the bloggerworld. But who knows, maybe in a week or two, i might be more open about these. For sure, as i start jumping over life's hurdles ill be more open. Or better yet, even before that! Carpe diem!

and i'm back..!!!

hey i missed this place! it has been a while though. actually I was shocked when i noticed that my last post was JUNE 14, 2009..! that's almost 3 years..! wheeew!

well, well now i'm back and pretty much in good shape to start things all over again..! and thanks a lot to my cousin anniniput, who gave a hand in repairing my long lost blog. actually my email add had been long forgotten...!

well, to share something about the three years, i remember the last time i blogged was months after my graduation... hadn't had a job back then! now? i had found a job! i mean, i had two jobs now since then.

i now work in a local government unit... but i'm planning to look for another one within this year... the stress i get there takes a toll on my physical wellness and on my sanity.! i swear its true..!

i'm on a job hunt now... a job in which i don't have to spend 8 hours a day... a job that would let me serve Jehovah ...

any job ideas there? i'm sure it'll help

for almost 3 months i have been waiting for the Career Service Examination Result...

A total of 5,998 examinees passed the career service examinations administered nationwide last March 29, the Civil Service Commission (CSC) reported. This as the CSC reminds interested applicants of the July 26 Professional and Sub-Professional written examinations that applications will be accepted until June 10 only.Said number of passers comprises 10 percent out of the 60,009 examinees who took the March 29 exam. Of the figure, 4,312 or 71.89 percent were successful examinees of the Professional level while the rest were passers of the Sub-Professional test.

Jose Rafael D. Neme from National Capital Region (NCR) topped the Professional level with a rating of 90.68, while Romina Shayne T. Gabriel, also from NCR, topped the Sub-Professional level with a 92.68 rating.According to CSC, the new Professional and Sub-Professional eligibles can be appointed to second level positions and first level positions, respectively, in the government that are not covered by Bar, Board or other special laws, provided they meet the education, experience and training requirements of the positions.

i had mah hair cut Thursday this week...

after 6yrs, its short again...!!

schooling days are over... a new chapter unfolds, i have to find a job now... not like before i just leave the earning to my parents... and juz wait for my allowance...!! but now, things are different, i have to earn for myself and for my family as well...connection with mah new hair-cut..???

hmmf ill share to you, a little secret....cutting my hair very short, had always been my way of telling people that im ready and willing to forget things that should and must be forgotten... like the hair that has been cut-off, memories and feelings are left behind...

but now, its not that i want to forget something, its more of welcoming new things to come mah way.. bagong buhay, kumbaga...!!hihi

yeah bah... new hair cut, for a new chapter of my life...!!!
